What does Protein and Blood in Urine Mean for Henoch-Schonlein Purpura (HSP)

2012-10-17 17:01

Henoch-Schonlein Purpura (HSP) is a capillary abnormalism hemorrhagic disease which is associated to capillary autoimmune injury. Typical symptoms of the disease include skin purpura, joint pains, abdominal pains, etc. For the patients, presence of protein and blood in urine suggests involved damage in the kidneys. The kidneys are main filtering organs of the kidneys that will present leakage of blood and proteins. When HSP damage the kidneys, abnormal urine changes through naked eyes or urinalysis can be detected out.

Kidney disease caused by Henoch-Schonlein Purpura is called as Purpura Nephritis. The incidence of Nephritis in HSP patients is high. According to studies, symptoms of kidney disease can occur at any stage of HSP- most commonly four weeks after one gets Henoch-Schonlein Purpura, but can also happen months to over 2 years after onset of HSP.

In Purpura Nephritis, both gross and microscopic hematuria can occur; gross hematuria is more common in children sufferers. Protein in urine is often moderate, not necessarily in proportion to the severity of hematuria. You may wonder how Henoch-Schonlein Purpura (HSP) would damage the kidneys and cause blood and protein in urine. Let’s originate from how Purpura Nephritis occurs.

Henoch-Schonlein Purpura (HSP) is characterized by systemic capillary damage due to allergy to certain substances. Hemorrhage can occur in various organs, such as skin, mucosa, joints and cavity. Permeability on blood vessels is increased, and antigen-antibody immne complex can activate immune cells and activate infiltration of inflammatory factors to cause tissue damage. Purpura Nephritis shares the same pathogenic basis with HSP. It is accepted that occurrence of Purpura Nephritis include a process of formation, circulation and deposition of immune complexes. Elevated levels of immune complex, especially IgA immuglobulin, can be detected. The immune complex deposition on glomerular cells damage normal filtration functions of glomerulus and cause an abnormal loss of proteins and red blood cells from urine.

From the above, we can conclude the reason why people with HSP have blood and protein in urine is that their kidneys have been damaged. To treat the symptoms, firstly the abnormal allergic damage to kidneys from HSP should be stopped. Regulating the immune system and improving injured renal cells should also be combined to rid the symptoms effectively and stop further renal fibrosis.
