Prognosis in Membranous Nephropathy

Membranous Nephropathy is a latent disease. It is often diagnosed when an abnormal result is found during routine physical tests. The patients are quite uneasy because they don’t know much about this disease and whether the disease is severe or not. What about the prognosis in Membranous Nephropathy? Membranous Nephropathy is a kidney disorder wherein glomerulus, the filtering units of the kidneys gets damaged. In consequence, a higher amount of proteins will be present in urine. The prognosis for Membranous Nephropathy varies from case to case. In approximately 25% cases, the disease gets remitted spontaneously. About half of the patients experience microscopic blood in urine, however, heavy microscopic urine blood is not a characteristic symptom in Membranous Nephropathy. About 17% to 50% adult patients have high blood pressure from initial disease onset. Usually, those who have damaged kidney function and hypertension at initial course have a poorer prognosis than others.

In addition, the degree and duration time of proteinuria are also closely associated to the prognosis in Membranous Nephropathy. Male gender, an older age, renal insufficiency and severe glomerulosclerosis as well as renal tubular injury indicates a poorer prognosis.

Membranous Nephropathy tends to have a slow progressive deterioration process. The patients may have asymptomatic proteinuria which lasts for 2-13 years before developing into Nephrotic syndrome, and glomerular filtration rate still declines slowly progressively. Some may take five years, others may take 20 years before progressing into end-stage kidney disease.
