Q:

I have type 2 diabetes for about 8 years and now I suffer from kidney problems. The blood test that i done last time shows that my serum creatinine level is slightly higher, about 3.1. The doctor said that i am now in stage 2 chronic kidney disease. I am now worried and want to know how quickly will stage 2 CKD progress into the end stage.

A:

At present, your illness condition is not very severe, so do not worry that much. Chronic kidney disease can be divided into 5 stages. Stage 2 CKD is mild renal damages and decline of renal functions.

Since you have diabetes for 8 years, your kidney damages are caused by your long term high blood sugar level, therefore the first step and also the key point is to bring your blood sugar well under control so as to prevent further damages to the kidneys and other organs. So long as the underlying cause is treated well, renal damages will be reversed and you can stay in this stage for years without further progressing into more advanced renal damages.

Diet adjustment and regular proper exercises can help better control blood sugar. When blood sugar level is hard to treat with mere diet and exercises, your doctor may prescribe some orally intake or injection of hypoglycemic drugs.

Thought it is difficult to recover functions of the damaged renal tissues, so long as further renal damages are prevented, your current kidney functions (more than half of kidney functions are normal) can ensure normal life.

However if you do not pay much attention to your daily life, kidney functions may experience rapid decline.

In a word, how quick the progression from stage 2 to stage 5 will be depends on weather you take timely and proper treatments and your lifestyle.

Besides, take regular tests and have close monitoring of your kidney functions. Once there is clinical proteinuria (urinary protein is more than 0.5g/d), renal functions will experience irreversible decline and the average life expectancy since proteinuria is about 10 years.
