Q:

My 24h Urine Protein is 3525mg. Blood pressure is 135/85 on average. The following are latest test results:

Fasting blood glucose 116

Creatinine 1.11, blood urea nitrogen 14, total blood cholesterol level 373(125-250) and calcium level 7.2.

I am taking omnacartil candace pan, lasix and prednisone. What do you suggest for my condition?

A:

You told me your 24 hours urine protein is 3525mg. I think you have Nephrotic Syndrome. What's your serum albumin? If your serum albumin is low, you'd better eat more protein. If your serum albumin is still normal, low protein diet is right. You said you are taking lasix. Do you still have swelling? When did you find your kidney problem?

The features of Nephrotic Syndrome are high blood lipid, high blood pressure, large amount of protein leakage and low protein in blood. In order to stop the protein leakage, hormone medicine is often used. The abnormal immune reaction will cause much immune complex depositing on the glomerular basement membrane and will cause the proliferation and swelling of mesangial cells thus damaging the charge barrier and mechanical barrier. It cannot block the leakage of protein and red blood cells. In treating this disease, we have to first control the immune reaction. Most immune blockers can achieve this effect. However, there is no effective western medicine that can remove the immune complex. If the immune complex remains in the body, whenever we reduce the dose of medicine or stop it, the symptoms will relapse. Only if we remove the immune complex on the basement membrane and repair the damaged mesangial cells, the protein leakage can be stopped fundamentally. Chinese medicine can regulate the immune reaction and rebuild immune system, so if well treated the disease will not recur.

As of now, if your blood sugar is controlled stable at 90, it is good. Because long time use prednisones may cause Diabetes. Your blood pressure is also ok, but not at an ideal level. You can do proper exercise like walking after dinner. Ramipril is for high blood pressure.

Your cholesterol is still very high. It is 373 while the normal range is 125-250. I suggest you take Lipitor for it. High blood lipid will cause blood high viscosity syndrome and even blood stasis. But you didn't take any treatment for your blood lipid.

Your calcium is low. It is due to use of prednisone.

Your kidney function is still good. What is concerned is still the protein leakage and swelling. Your protein in blood is too low. I suggest you take high protein food. There is already mild ascites. If you don't increase protein intake, it will be more serious and even need surgery to remove the liquid in abdomen.

Hope the above explanation is of help for you. For any more unconcerned question, you may consult me directly.
