Q:

My daughter has had type 3 Diabetes for 3 years. She has foams in urine which is diagnosed as proteinuria by doctor….. blood glucose is out of control… Is it kidney disease? Will the proteinuria go away?

A:

As far as I am concerned, it is Diabetic Nephropathy that is causing the proteinuria. Diabetic Nephropathy is kidney disorder which may result from poor controlling of both type 1 and Type 2 Diabetes. The kidney filter, known as glomerulus, filtrates bloodstream and stops proteins from escaping into urine. Diabetes can damage glomerular filtration membrane. If glomerular cells are impaired, a higher amount of proteins will be present in urine. In Diabetic Nephropathy, the amount of protein in urine increases with worsening of kidney damage.
You mentioned of the proteinuria, what about the 24h urine output? Microalbuminuria—slight higher albumin in urine, is the earliest manifestation of diabetic kidney disease. The foams in urine means a severer stage of diabetic kidney disease. Careful blood sugar management will help to reduce progression of kidney damage. The blood pressure should also be well managed well. Proteinuria should be treated timely now, since the protein loss may worsen renal interstitial fibrosis.
