PKD, Creatinine 4.8, urine output 2500, back pain and liver cyst

Hi, I have PKD and it was diagnosed 8 years ago because my father has PKD and I had myself tested and it was confirmed. I am a female and now 50 years old. The largest cyst is 5cm, creatinine is 4.8mg/dl and urine 24-hour urine output is more than 2500ml. I constantly suffer from back pain and I also have cysts on my liver. What should I do and how long will I live with PKD?
