Diabetic, Creatinine 2.1, Potassium 7.2, Leg Swelling

Hi, my dad is a diabetic patient since 20 years. Right now his creatinine level is 2.1 and potassium was 7.2 two days back. He has swelling in his right leg. Can you please tell me what is the position of his health condition and what will be the best treatment for him. Please advise me ASAP.
