The four categories kidney diseases that can cause back pains:

Kidney enlargement due to renal parenchyma diseases. This is most common among acute glomerulonephritis and acute progressively nephritis. The pain is due to stretching of the renal capsules due to continuous enlargement of the kidneys. Besides back pain, some patients may also have gross hematuria, edema, high blood pressure, etc. One thing needs to be noted that pain-killer has little use for this kind of back pain.

Infections and inflammations in the kidney. The main reason is bacterial infection and back pain is the early symptom of renal abscess and acute pyelonephritis. The pain is usually in one side and will become unbearable when pressed. Usually fever and chill will be accompanied and the examinations of routine blood test and B ultrasound can detect it.

Tumor or cysts in the kidneys. Common renal cystic diseases include kidney cysts, polycystic kidney disease, and tumors weather benign or malignant. If the tumor or cysts continue to grow and stretch the capsule, it will cause persistent dull pain. B ultrasound and CT scan can confirm it.

Kidney stones. If the stones are located in the ureter, there will be renal colic. The pain is usually sharp and intermittent. Patients will have nausea, vomiting and in severe conditions gross hematuria.

Of cause not all back pain case is due to kidney diseases and there are many other reasons resulting in back pain. For example, bone diseases, strain of lumbar muscles and gynecological diseases.
