Renal Parenchymal Disease and High Blood Pressure

High blood pressure is a common symptom of renal parenchymal diseases. It is called renal parenchymal hypertension and it accounts for 5-10% of all hypertension cases in clinic.

Renal parenchymal disease is not a single disease but includes a group of kidney problems such as glomerulonephritis, chronic pyelonephritis, polycystic kidney disease, urinary tract obstruction, etc. Nephritis patients have 34-80% incidence to develop high blood pressure, PKD patients have 30-46% incidence and chronic pyelonephritis patients have 30% incidence to develop renal hypertension. High blood pressure will become worse with progressive worsening of illness conditions and about 90% ESRD patients will develop high blood pressure. In most cases, the high blood pressure is caused by increased secretion of renin.

Renal parenchymal hypertension has 2 times higher incidence to develop into malignant hypertension which is more common among IgA nephropathy patients. A study has found that the survival rate within 5 years for primary malignant hypertension patients is about 60%, while for patients with renal parenchymal malignant hypertension, the survival rate within 1 years is only 4%. Therefore we can see that early and effective treatment for renal parenchymal diseases and high blood pressure is very important for the patients.

Treatments for renal parenchymal hypertension are as follows:

1. Control salt and water intake. This can help reduce blood volume and help control high blood pressure.

2. Diuretics. Diuretics can help remove excess water and sodium retention in the patient’s body.

3. ACEI and ARB drugs. They are widely used in clinic to help reduce high blood pressure. These drugs can help ease glomerular internal pressure and reduce damages to the glomeruli.

4. Micro-Chinese Medicine Osmotherapy. It is based on Traditional Chinese Medicines and it can help lower high blood pressure, repair damaged renal parenchymal tissues and improve kidney functions.
